By submitting this form, you give permission for the child(ren) listed above to be transported to and from Calvary Baptist Church and to participate in all church activities. If a child expresses a desire to trust Christ as their personal Saviour or to be baptized, our workers will carefully explain these decisions from the Bible. Salvation and baptism are never forced or pressured and are only discussed with children who express a personal desire to learn more.

You agree not to hold Calvary Baptist Church or its officers liable for any harm to your child. Please ask a church representative if you have any questions regarding our beliefs.

We are excited to have your child ride our church van and be a part of Calvary Baptist Church! After submitting this form, one of our workers will contact you to confirm your information and answer any questions you may have. Our goal is to provide a safe, fun, and Christ-centered environment where every child is loved and taught God's Word. During each service, children enjoy age-appropriate Bible lessons, games, songs, and activities. We look forward to meeting your family and helping your child grow in the Lord.
